Service Provider Criteria and Contract Requirements
Rosemead School District wishes to take advantage of E-Rate funding discounts for E-Rate eligible services.
The District’s decision to undertake these projects and services depend on funding from the E-rate program.
The successful bidder(s) shall be responsible for the following”
• Properly participating in the Federal E‐Rate program as a Service Provider.
• Make themselves thoroughly familiar with any current and future rules or regulations regarding the
E‐rate program.
• Provide a valid SPIN (Service Provider Identification Number) at the time the proposal is submitted.
• Provide a Federal Communications Commission (FCC) Registration Number with its proposal.
• Agree that all contracts entered into as a result of this RFP will be contingent upon the specific
funding of the funding request (FRN) and the approved application’s E‐Rate discount.
• Accept that the maximum percentage the District will be liable for is the pre‐discount amount minus
the funded amount as shown on the Form 471 Block 5.
